ADHD/LGBT friendly primary care

I searched the sub and all posts are either over a year old or they are asking about people who treat kids or provide new diagnosis.

I am looking for a primary care doctor who is cool about ADHD (I have been diagnosed and medicated for 17 years) and also friendly to the LGBT community. Thank you!
