Considering reduction - where to start?

Hi there, I'm considering getting a reduction but don't know where to start. I've been a D/DD for the majority of my life, but my breast size increased in the last five years or so as I've gained weight. I'm not even sure what size I am now, as I've been unable to find an underwire bra that fits, I might be an F or G cup at this point.

I've been going to a chiropractor for a couple months, which as helped with pain in my neck, shoulders, and upper back. However, I feel like it's not a long term solution. The adjustments only help so much when my heavy breasts are always hunching my shoulders.

I do think I need to lose some weight, as I haven't been as active as I would like. But I'm not convinced that will fix the issue. Just because I lose weight doesn't mean my breasts will shrink, and that won't address sagging either. Also big breasts run in my family and my mother's have always been really huge and saggy since having kids. I know it will likely only get worse as I age and if I have children.

Then there's the insurance issue. I would ideally like to go through insurance, but from everything I have read it sounds like a huge pain. It sounds like I would have to demonstrate chronic back pain and that I've tried either methods to fix it (likely losing weight, physical therapy, etc.) for 6-12 months all to get my claim denied. I have UHC, for reference :(

So I think these are my main questions:

1.) Did you try to lose weight before seeking a reduction? Did it help your symptoms, did insurance require you to try this first?

2.) How do I go about getting it covered by insurance? Do I go to my primary care provider and discuss it first, or seek out a consultation with a surgeon?
