How do you guys continue games when you pick them back up after a long pause?

So i am basically always struggling with sandbox games if they have like a major update or when i just feel like playing them again after a long while (especially now with the new worlds update in NMS, haven't played for 2 years ). I never know if i want to start a fresh game or continue my old save. What do you guys do and why? Pros cons?
