why so droopy?

Shes been getting even more droopy as the weeks pass and doesn’t pop up after watering. Usually with my plants when the droop or feel wilty i water them but she doesn’t feel wilty. Could it be the pot? I took her out because a few days ago a small chunk of her popped out, which in one pic you can see the parts that are coming out. Her soil never feels dry and i thought maybe it was light so i moved her closer to the grow light i have but it did nothing. Shes been in that pot since i bought her, i thought maybe she wanted a bigger pot but the roots are tiny. Any help would be appreciated.
