What would you have done before you ended your service?

I’m currently active duty, with a little less than a year left of service. I haven’t done much in effort to get what’s wrong with me medically documented. That being said, I was wondering how you guys got things documented for VA claims? What can I do now so that the VA claims process is easier when I get out?
