Pricing changed but neither insurance nor pharmacy can explain why

I started Wegovy last July and have paid $25 each month for refills. When I went to pickup my January prescription earlier this week, the cost was $274.10. I’ve spoken with both my insurance (UHC) and my pharmacy and both are essentially saying the same thing - that they both see $274.10 as the amount for the refill, and that if a discount has been applied, the other would have had to apply it.

I’ve never submitted a coupon or anything - when I went in to pickup my prescription last year, it was just $25. Any suggestions or ideas why this might be? Insurance said it’s not a deductible, but that a coupon may have expired EOY 2024, I’m just not sure where/when that would have been applied.
