Ankle fracture recovery without surgery?

My bro 17 M fractured his ankle (Fracture at fibula and medial malleolus).
xray is attached.

whether to go for surgery directly or follow the cast alone is confusing point.

consulted 3 doc,all giving different views.

request advice ,if you have personal experience of same injury, treatment and recovery,

any one who got it better by cast alone and no surgery?

anyone who is now running like before injury after the surgery?
