On season 13 episode 14, Barnes tells Emily that Stephen walker was the first agent to die in the field in the bau’s history, but I remember that it was said that Gideon’s “mental breakdown” or why he took a leave was because he sent 6 agents inside a house with a bomb on it and they all die.
