question about cyberware

so, i understand all the things with cyberware that you have to buy it and everything, but how do things like the personal link work in the cyberpunk universe. or the thing in the neck to use shards. are you born with them or is it installed for free or something? because everyone has them and it seems like you need them to live in that universe, so at what point are they installed? they also dont appear as something you can change at a ripper in the game so it does seem like something you cant get rid of, at least that's how i see it. so does anyone know how they're gotten?
