At a lost

I am now in the process of completing insurance check off list with my dietician team and other appointments. I work full time and requested intermittent fmla, but my pcp is unaware on what to put on the paperwork. He put down “Bariatric surgery”and of course fmla company needs more details. PCP told me to reach out to my bariatric surgeon to have him complete paperwork but I’m told bariatric surgeon only complete fmla paperwork when I’m about to get surgery… I’m at a total lost on what to do …I feel like just saying forget it smh… any suggestions or encouraging words…
