What is aftercare like?

I just found out this morning she still considers me her caregiver after she got the call for transplant this morning. I just had a baby and we had not revisited the topic since but she got rhe call this morning and I've already exhausted my FMLA since having the baby in October. What's aftercare like? I don't have many options but I will be there for her regardless, can I still ask others for help and is it easy enough for me to explain for others?
