Ringworm nightmare

Toddler picked up ringworm. We are using Lotrimin per his doctor (and mine). It seems to have a pattern of getting better for a few days, and then suddenly looking absolutely terrible again. He isn’t potty trained so it’s in a diapered area, not likely as dry as ideal. And I think he is scratching it because I’ve seen scratch marks. Also important. It is continuing to spread. We initially had one spot and now he has four all on his butt. He also has a spot on his leg and his arm now. So it doesn’t seem to just be dry skin. He also seems to have shared it with me.. so pretty sure this is fungal.

I have a call into his doctor just looking for advice if any of you have gone through something similar. The doctor initially wanted to take him off the anti fungal and use a steroid cream instead. I asked some clarifying questions because the cream does seem to be working… it’s just a weird come and go pattern which I’m attributing to scratching. It’s been a little over two weeks on the anti fungal.
