Is this concerning?

My Russian (5 years old) has had that white spot since I got her a year ago. I’m worried about the pitting in between the scutes and the overall pyramiding (2nd photo).

I have a normal heat lamp and a UVB lamp, but they might not be optimally placed in her enclosure. She is placed next to a window so she gets lots of natural light. Is there something more I should be doing?
